2. Download and print a copy of the
PARENT/GUARDIAN AGREEMENT--STUDENT HEALTH FORM,
and mail it to us along with your deposit of $100.



Adult Students
Send the abbreviated EMAIL INFO FORM;

IF YOU ARE NOT ABLE TO APPLY ONLINE:

Please send us an email requesting printed copies of the
Enrollment Form and the Parent/Guardian Agreement--Student Health Form.
